Job Description
We are seeking a Temporary MLTC Nurse Care Manager for a short-term assignment supporting Managed Long Term Care (MLTC) operations. This role is ideal for a Registered Nurse with strong experience in care management, UAS assessments, and MLTC services.
Assignment Details:
- Duration: Approximately 90–120 days (potential to go longer)
- Schedule: Monday to Friday, 9:00 AM – 5:00 PM
- Location: Field work primarily in the Manhattan area
Responsibilities:
- Conduct and complete care plans
- Perform monthly member outreach calls
- Follow up on member needs and services
- Conduct field visits and assessments as required
Qualifications:
- Current New York State RN license and registration required
- Bachelor of Science in Nursing (BSN) preferred; Associate’s Degree in Nursing accepted with a minimum of three (3) years of RN experience
- Prior experience in Care Management, UAS, and MLTC is essential
- Bilingual in Mandarin or Cantonese is required
