Job Description

Admissions Evaluator / Registered Nurse / RN
Upper West Side, Manhattan
An established organization dedicated to helping older New Yorkers live full, meaningful, and dignified lives is seeking an experienced Registered Nurse (RN) Admissions Evaluator to join their Admissions team. This is an excellent opportunity for an RN who is passionate about elder care, strong in clinical assessment, and skilled in building relationships with hospitals, care teams, and families.


Position Overview
The RN Admissions Evaluator plays a key role in assessing clinical appropriateness for admission, coordinating with referral sources, and supporting families throughout the admissions process. This role requires strong clinical judgment, excellent communication skills, and the ability to thrive in a fast-paced environment.


Key Responsibilities

  • Review PRI’s and all supporting clinical documentation to evaluate appropriateness for admission.
  • Serve as a liaison with hospitals throughout the NYC metropolitan area—conducting field visits and telephone consultations to evaluate potential applicants.
  • Complete all required clinical pre-admission assessment forms and gather additional medical/social information for a thorough assessment.
  • Begin the application process during field visits and phone consultations, sharing information about available programs and obtaining psychosocial or financial information when needed.
  • Document all applicant clinical and psychosocial assessments thoroughly.
  • Submit monthly statistics and reports as requested by the Director of Admissions.
  • Contact Managed Care plans to obtain pre-authorizations and review level-of-care requirements.
  • Conduct facility tours with potential patients, families, and representatives.
  • Collaborate with HMO Case Managers to review skilled needs and optimize appropriate levels of care.
  • Participate in weekend and holiday admissions coverage as needed.
  • Perform additional duties as assigned.

Qualifications

  • Registered Nurse (RN) licensed in New York State.
  • MDS experience and PRI certification preferred.
  • Prior experience in post-acute admissions strongly preferred.
  • Acute Care experience is a plus.
  • Bilingual abilities (English/Spanish) a plus.

Specialized Skills & Competencies

  • Professional, courteous, and responsive communication (in-person and by phone).
  • Strong writing and clinical assessment skills.
  • Ability to build positive working relationships with referral sources and internal staff.
  • Ability to multi-task and maintain accuracy in a fast-paced environment.
  • Proficiency in Electronic Medical Records, Microsoft Outlook, Word, and Excel.
  • Clear and effective verbal and written communication skills.
  • Bilingual (English/Spanish) a plus.

Additional Skills

  • Ability to relate effectively and respectfully to adult and geriatric populations.
  • Strong understanding of medical terminology and conditions relevant to adult/geriatric care.
  • Ability to complete thorough bio-psychosocial assessments and determine appropriate placement within the continuum of services offered.
