Director of Patient Services, Homecare RN – HAPI
Home Assistance Personnel Inc. – Bronx, NY
Nursing – Full Time – Salary Range $120,000 to $140,000

As a comprehensive nonprofit health system, the organization serves people of all backgrounds, offering skilled nursing, short-term rehabilitation, adult day health care, home care services, and a Medicaid Assisted Living Program.
The Home Assistance Personnel Inc. (HAPI) division has extensive experience delivering rehabilitation and home care services, with a mission to help each patient regain independence and maintain optimal health as quickly as possible.
The HAPI Division is seeking a full-time Director of Patient Care Services to oversee clinical operations, staff development, quality improvement initiatives, and compliance for its home care programs.
Job Summary:
The Director of Patient Care oversees all clinical operations, performance improvement initiatives, training programs, medical compliance, and supervision of nursing and field staff.
Reports to: AVP, Home & Community Based Services and/or CFO
Key Responsibilities:
Clinical Program Oversight

Develop and manage a comprehensive clinical care program.

Participate in policy development, updates, and revisions.
-
Interpret and communicate agency policies to staff, clients, families, and community partners.
-
Ensure timely and thorough client assessments at admission.
-
Confirm admission eligibility based on agency criteria.
-
Guarantee development and implementation of individualized plans of care.
-
Coordinate all services to ensure safe and effective care delivery.
-
Ensure staff compliance with federal, state, and local regulations.
-
Cover field nursing duties when needed.
-
Maintain patient records in accordance with regulatory standards.

Provide supervision to nursing and paraprofessional staff.
Quality & Performance Improvement
-
Lead quality program activities and performance improvement strategies.
-
Develop, evaluate, and refine quality and safety initiatives.
-
Create quality metrics and support compliance oversight.
-
Serve as a subject matter expert for quality management.
-
Implement evidence-based practices that meet CMS and DOH standards.
-
Oversee incident/accident reporting, complaints, and infection control compliance.
-
Review aggregated data and performance trends; prepare and present reports.

Develop performance improvement education and orientation programs.
Staff Orientation, Training & Competency
-
Oversee orientation and annual competency assessments of all field staff.

Ensure in-service programs meet regulatory and accreditation requirements.

Monitor field staff through supervisory visits and competency checks.

Develop clinical education and training programs.
Health Status Monitoring
-
Ensure staff meet all health requirements prior to employment and annually.

Provide health counseling and exposure management as needed.
Conflict Resolution & Staff Support

Investigate complaints, incidents, and root causes.
-
Provide staff counseling and progressive discipline as appropriate.

Implement corrective action plans in a timely manner.
Customer Service & Patient Experience
-
Ensure timely case acceptance and service delivery.
-
Ensure satisfaction surveys are completed and addressed.

Promote a culture of excellent customer service among all staff.
Operational & Cost Efficiency
-
Review operations to reduce duplication and waste.
-
Optimize staffing patterns in collaboration with leadership.
-
Monitor cost efficiency in procedures, systems, and supplies.
Leadership, Committees & Projects
-
Attend mandatory meetings, in-services, and care conferences.
-
Participate in quality and performance committees.
-
Lead quality improvement initiatives across product lines.

Develop strategic plans for improved clinical outcomes and regulatory compliance.

Evaluate and oversee data collection, analysis, and reporting.
Qualifications:
-
Current RN license in New York State.

Bachelor’s degree in Nursing required.

Minimum 3 years of clinical supervisory experience.
-
At least 1 year of home care experience.
-
Valid NYS Driver’s License and access to a vehicle.
Compensation & Benefits:

Salary Range: $120,000 – $140,000 annually
