Job Description
Nights Hospice Field Case Manager (RN)
Shift Options: 5:00 PM–1:00 AM or 1:00 AM–9:00 AM
Compensation: $120,000–$130,000 + $15,000 sign-on bonus + comprehensive benefits package

Our multidisciplinary teams deliver compassionate, culturally sensitive end-of-life and palliative care to patients and families across home, assisted living, skilled nursing, and hospital settings.
Responsibilities
In this field-based role, the Hospice Field Case Manager will:

Conduct comprehensive physical and psychosocial assessments and document visits electronically.

Provide patient care in both community and facility settings.

Develop, implement, and modify individualized nursing plans of care based on patient needs.

Collaborate with physicians to obtain orders and coordinate plan-of-care updates.

Supervise, train, and evaluate home health aides and homemakers supporting hospice patients.

Participate in weekend coverage (one weekend per month) and serve as backup on-call 2–3 times per month.

Utilize provided car and escort services during backup on-call shifts for safety and convenience.
Qualifications

Graduate of an accredited School of Nursing (BSN preferred).

Active New York State RN license.

Minimum of two years of community-based or oncology nursing experience.

Valid New York State driver’s license.

Proficiency with Microsoft Word and Excel.

Bilingual skills strongly preferred.
