Job Description

Nights Hospice Field Case Manager (RN)
Shift Options: 5:00 PM–1:00 AM or 1:00 AM–9:00 AM
Compensation: $120,000–$130,000 + $15,000 sign-on bonus + comprehensive benefits package

Our multidisciplinary teams deliver compassionate, culturally sensitive end-of-life and palliative care to patients and families across home, assisted living, skilled nursing, and hospital settings.

Responsibilities

In this field-based role, the Hospice Field Case Manager will:

  • Conduct comprehensive physical and psychosocial assessments and document visits electronically.

  • Provide patient care in both community and facility settings.

  • Develop, implement, and modify individualized nursing plans of care based on patient needs.

  • Collaborate with physicians to obtain orders and coordinate plan-of-care updates.

  • Supervise, train, and evaluate home health aides and homemakers supporting hospice patients.

  • Participate in weekend coverage (one weekend per month) and serve as backup on-call 2–3 times per month.

  • Utilize provided car and escort services during backup on-call shifts for safety and convenience.

Qualifications

  • Graduate of an accredited School of Nursing (BSN preferred).

  • Active New York State RN license.

  • Minimum of two years of community-based or oncology nursing experience.

  • Valid New York State driver’s license.

  • Proficiency with Microsoft Word and Excel.

  • Bilingual skills strongly preferred.
