Overview:

Brand new state of the art facility offering a fresh approach to rehabilitation and long term care. From the first day of admission, the staff makes sure each resident and family member feel comfortable, content and secure. Our programs and services provide comprehensive care for our residents, including short-term care, long-term care, tracheostomy care, wound care, hospice and palliative care.

Responsibilities:

• Provide supervision and leadership to nursing staff.

• Ensure high-quality care is delivered in compliance with medical and regulatory standards.

• Coordinate and implement individualized care plans for residents.

• Monitor residents’ health and progress, intervening as necessary.

• Communicate effectively with residents, families, and interdisciplinary teams.

• Oversee medication administration, wound care, and specialized treatments (e.g., tracheostomy care, hospice).

• Participate in performance improvement initiatives and staff training programs.

• Maintain accurate documentation and reporting in electronic medical records.

Qualifications:

• Current RN license in good standing (required).

• New graduate RNs are welcome – training and mentorship will be provided.

• Strong organizational, communication, and leadership skills.
