Job Description

Community Health Nurse / Field RN / LHCSA
Location: Brooklyn and Queens NY
Employment Type: Full-Time | Permanent | Monday–Friday, Business Hours
Salary: Up to $113,000 annually

A licensed home care services agency is seeking a Community Health Nurse (RN) to join its dynamic and compassionate home care team. This is a full-time, permanent position offering a competitive salary and a Monday–Friday daytime schedule—no evenings or weekends required.

This role plays a vital part in delivering high-quality care to members in the community and ensuring compliance with care plans and clinical standards.

Key Responsibilities:

  • Conduct in-home assessments including Start of Care, reassessments, post-hospitalization visits, SCIC visits, and UAS assessments as required.
  • Complete documentation of visits within required timeframes (e.g., within 3 days of visit).
  • Provide orientation and supervision to home health aides on Day 1 of care and throughout the case.
  • Perform comprehensive clinical and environmental assessments; consult with physicians on any changes in patient status or treatment plans.
  • Identify the need for referrals to additional disciplines (e.g., PT, OT, MSW, speech therapy, nutrition).
  • Establish and review Plans of Care (POC) with patients and caregivers; instruct and supervise aides accordingly.
  • Reinforce emergency plans and assess patient’s medication regimen and understanding.
  • Maintain accurate and timely documentation in accordance with all regulations.
  • Act as a liaison between patients and the Care Management Team.
  • Precept and support new nurses as needed.
  • Attend all required meetings and in-services.

Qualifications:

  • Current NYS Registered Nurse (RN) license
  • Graduate of an approved nursing program
  • UAS certification or willingness to obtain
  • Prior home care experience strongly preferred

Compensation and Benefits
