Hospice Field Admissions Nurse (RN)
Compensation
Salary Range: $112,000 – $130,000 per year
Bonus / Commission (if applicable):
This role is eligible for a sign-on bonus of $15,000, paid over 3 years.
Location: Brooklyn, Queens, and Nassau County, NY

Position Summary
As a Hospice Field Admissions Nurse (RN), you will provide compassionate, patient-centered care by performing comprehensive assessments for individuals entering hospice services. In this field-based role, you will evaluate physical and psychosocial needs, develop individualized care plans, and support patients and families during end-of-life care. You will collaborate with a multidisciplinary team to ensure high-quality, culturally sensitive care across home, hospital, and assisted living settings.

? Key Duties:

• Conduct initial physical and psychosocial assessments for hospice eligibility in home, hospital, and facility settings
• Develop individualized plans of care and recommend appropriate hospice services
• Admit patients into hospice care following NHPCO standards and regulatory guidelines
• Provide education and support to patients, families, and caregivers regarding hospice services
• Engage in community outreach with physicians and providers to support program growth and quality improvement

Requirements
? Education

• Required: Graduate from an accredited School of Nursing
• Preferred (if applicable): Bachelor of Science in Nursing (BSN)

? Licenses & Certifications

• Required:
  • Active New York State Registered Nurse (RN) license
  • Valid NYS Driver’s License

? Skills:

• Strong clinical assessment and care planning skills in hospice or related settings
• Excellent communication and patient/family support skills
• Ability to work independently in a field-based environment
• Proficiency in Microsoft Word and Excel
