Job Description

RN Case Manager – MLTC – Remote with One Field Day
Opportunities in Brooklyn, Bronx, Queens, and Nassau/Suffolk

Position Summary :

  • Overall, responsible for the care management of a defined population with the plan; within that, responsible for care planning that is of good quality and appropriate to the needs of the members, for determining that members receive the services they need, and that services are acceptable and accessible to the member; assures that the care plan changes as members’ needs change.
  • Evaluate clinical assessments in developing the care plan and surfaces issues not readily resolved to the AVP for the region and the Intensive Care Management process.
  • Oversees, develops and approves the care plan and other disciplines of the MLTC member.
  • Re-evaluates clinical assessments and makes any necessary revisions to the care plan. Responsible for presenting at risk members to Intensive Care Management.
  • Makes home visits  (one field day, one borough, 4-6 visits a week)

Qualifications:

  • Current New York State Registered Nurse License required. Bachelor Degree in Nursing or a Health or Human Services field required.
  • Two (2) years’ experience in Home care required.
  • Experience assessing patients for home care safety and developing appropriate care plans and plan adjustments required.
  • Prior supervisory experience preferred.
  • MS Office required.
  • Bilingual Language Skill Strongly Preferred.

Salary up to $115,000
