Position Summary
As a Licensed Practical Nurse (LPN) in a community health center setting, you will provide professional nursing care to a diverse patient population while supporting both physical and psychosocial needs. You will work collaboratively with providers and the multidisciplinary team to ensure high-quality, patient-centered care. This role emphasizes inclusive care practices, strong clinical support, and adherence to established healthcare standards and regulations.
Responsibilities

? Key Duties:
- Provide direct patient care including vital signs, assessments, medication administration, and treatments under RN supervision
- Assist with examinations, procedures, diagnostic testing, and maintain accurate patient records
- Educate patients and families on care plans, treatments, and self-care practices
- Maintain a safe, hygienic clinical environment and adhere to infection control protocols
- Collaborate with clinical team members and support patient flow, documentation, and care coordination
Requirements

- Required: Graduate of an accredited School of Nursing
? Licenses & Certifications
- Required:
- Current New York State Licensed Practical Nurse (LPN) license
- BLS CPR Certification
? Skills:
- Strong clinical and patient care skills in a ambulatory or clinic setting
- Excellent communication and interpersonal abilities
- Strong organizational and documentation skills
- Ability to work effectively within a multidisciplinary team
