Responsible for direct patient care, providing occupational therapy treatment to adult home care patients. Also
responsible for coordinating plan of care with Rehab management and submitting appropriate paperwork on timely basis
and abiding by all agency, state and federal rules, regulations, policies and procedures.
Conducts Start of Care (SOC) initial and routine home visit assessments and evaluations of patient’s therapy and healthcare needs and establishes an inclusive care plan.
Effectively case manages the care and services for assigned patients resulting in positive clinical outcomes.
Accurately and objectively documents comprehensive clinical findings and patient response to treatment.
Demonstrates expert knowledge unique to the practice of occupational therapy.

Qualifications:

• Bachelors of Science in Occupational Therapy from an accredited institute of
• learning, required; Masters degree, preferred
• Minimum of 1 year providing therapy treatment in a health care setting.
• Home Care experience preferred
• Current NYS license/registration in Occupational Therapy
• Knowledge of Microsoft Office applications, (MS Word, MS Excel, MS Outlook, etc
