Job Description
Day Shift (8:00 AM – 4:00 PM): On weekends
- Morning Routine and Medication Administration:
- Conduct morning assessments. LPNS must conduct rounds and ensure that all residents
- Administer all medications safely
- Refill medications and check for expired medications
- Ensure medication delivery. Document Medications on incoming medications log binder.
- Ensure proper documentation of medications administered and any observations.
- Document PRNS and the effect of PRN medications with approval from the RN
- Reassess individuals' PRN
- Progress notes on each shift
- Review bowel movement and sign
- Review Vital signs chart and sign
- Review turning and position and sign off
- Personal Care and Hygiene:
- Assist residents with personal hygiene activities such as bathing, grooming, and dressing.
- Monitor skin integrity and report any concerns to the Registered Nurse (RN) or healthcare provider.
- Any individuals with skin integrity concerns should have daily progress notes
- Meal Preparation and Nutrition:
- Conduct meal observations and complete the form.
- Monitor food intake and address any dietary restrictions or special needs.
- Ensure Fluid intake is done as required and documentation is completed.
- Activities and Therapy Sessions:
- Monitor turning and repositioning and ensure documentation
- Encourage resident participation and monitor for any changes in behavior or mood.
- Documentation and Communication:
- Maintain accurate documentation of daily activities, vital signs, and any incidents.
- Communicate effectively with other team members, including RNs, caregivers, and support staff.
- Report and Handover:
- Compile a detailed shift report summarizing resident activities, health status changes, and any incidents.
- Conduct a thorough handover to the Evening shift LPN/RN/Designee, including any updates or concerns.
- Review All flow sheets and sign off
Evening Shift (4:00 PM – 12:00 AM):
- Evening Medication Rounds:
- Conduct medication rounds and ensure all medications are administered on schedule.
- Monitor residents for any signs of adverse reactions and report to the RN or on-call physician if necessary.
- Conduct topical medications inventory, including checking for expirations.
- Assistance with Dinner and Evening Routine:
- Assist residents with dinner, meeting dietary needs and promoting a calm dining environment.
- Support evening routines including personal care, toileting, and preparing for bedtime.
- Ensure Fluid intake is done as required and documentation is completed
- Safety Checks and Environment Monitoring:
- Conduct safety checks throughout the residence, including bedrooms, common areas, and outdoor spaces.
- Check sleep charts
- Ensure equipment and emergency supplies are readily accessible and in working order.
- Behavioral Support and Crisis Intervention:
- Provide emotional support and behavioral redirection as needed.
- Implement crisis intervention techniques in collaboration with the RN and support team if behavioral challenges arise.
- Report and Handover:
- Compile a detailed shift report summarizing resident activities, health status changes, and any incidents.
- Conduct a thorough handover to the night shift LPN/RN/Designee, including any updates or concerns.
- Progress notes on each shift
- Review bowel movement and sign off
- Review the vital sign sheet and sign off
- Review All flow sheets and sign off
Night Shift (12:00 AM – 8:00 AM):
- Overnight Monitoring and Vital Signs:
- Conduct regular checks on residents throughout the night, including vital signs monitoring.
- Document observations and report any abnormalities to the RN or healthcare provider.
- Conduct medication inventories on all oral meds. Including refill and requesting new RX
- Comfort Care and Bedtime Routine:
- Assist residents with bedtime routines, ensuring comfort and security.
- Respond promptly to any resident requests or emergencies during the night.
- Documentation and Incident Reporting:
- Maintain accurate documentation of overnight activities, including any incidents, behaviors, or medical issues.
- Complete incident reports as necessary and communicate with the day shift team regarding any overnight developments.
- Review bowel movement charts and sign off on all flow sheets
- Ensure inventory of all medications supplies and feeding are at the residence.
- Check for medical appointment for the upcoming morning and ensure the referral is completed and is in the ready to go binder.
- Emergency Preparedness and Response:
- Be prepared to respond to medical emergencies or behavioral crises according to established protocols.
- Collaborate with on-call staff or emergency services as needed for resident care.
- Meal Preparation and Nutrition:
- Conduct meal observations and complete the form.
- Monitor food intake and address any dietary restrictions or special needs.
- Ensure Fluid intake is done as required and documentation is completed.
- End-of-Shift Reporting:
- Provide a comprehensive handover report to the early morning shift LPN/RN/Designee, highlighting overnight events and resident status.
- Ensure all documentation is complete and up to date for continuity of care.
- Complete progress notes each shift
General Responsibilities (All Shifts):
- Team Collaboration: Work with RNs, caregivers, therapists, and other healthcare professionals to ensure holistic care.
- Training and Development: Participate in ongoing training and professional development activities to enhance skills and knowledge in caring for individuals with developmental disabilities. Ensure training of new in services of PONS are reviewed and sign-in sheets are in place when applicable.
- Adherence to Policies and Procedures: Follow OPWDD guidelines, facility protocols, and regulatory requirements to maintain a safe and supportive environment for residents.
ALL LPNS are responsible for conducting reviews on all flowsheets including but not limited to bowel movement, seizure charts, sleep charts, vital signs, fluid intake, repositioning chart, and menses charts.
All abnormal findings will be reported to the RN for further directives.
The shift report will be made on the communication logbook and the communication logbook must be reviewed and signed off each day.
All contacts with RNs will be documented on the alert form. Statements will be collected if needed.
All consultants from clinics, urgent care, and Emergency room visits will be placed in the incoming medical binder for RN to review.
