LPN Supervisor (Evenings) – Assisted Living Program
Responsible for the provision of neighborhood leadership and social work services to resident and their families. The Program Nurse provides nursing care and services to residents in the ALR in accordance with the agency’s policies and procedures and under the supervision of a Registered Professional Nurse (Director of Nursing).
Responsibilities:
The Program Nurse will Assist with the preparation, implementation and continuing evaluation of the nursing care plan. Overseeing the units, assist with managing HHAs and daily tasks. Observes, records and reports to the appropriate person, resident’s symptoms, reactions and changes in the condition as indicated. Performs selected nursing procedures in keeping with licensure, such as, administration of medications and treatments. Manage resident medication: Medication Changes, New Prescriptions, Discontinued Medications, and Refills and obtain new prescriptions as needed, assist with admissions, conducting stand-up meetings with HHAs, conduct daily audits of all new admission and existing charts. Assist with incident reporting and follow-ups.
Qualifications:
Graduate of an accredited school of practical nursing with a valid and current New York State License and is currently registered as a licensed practical nurse pursuant to Article 139 of the New York State Education law. Minimum of one to two years as a licensed practical nursing experiences in community nursing, home care, acute care facility or adult care facility. Must have supervisory and medication oversight experience.
Compensation: $40 per hour
Monday – Friday with Rotating Weekends
