MLTC Intake Nurse – RN (Brooklyn, NY) – Chinese preferred – Hybrid

As a MLTC-Intake RN, you will be responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their family. 
 
Responsibilities:
The role of the Intake RN will be responsible for knowing require outreach activities within the community and the assessment of eligibility and safety of potential members. Collaborate with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members. Communicate with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.
 
Qualifications:
Must be a self-assured professional with a current New York State Registered Nurse License required. Bachelor’s degree in Nursing required. One (1) year experience in an acute care setting required. Must have UAS- NY experience.  Home care and management experience preferred. MS Office required. Effective oral, written, communication and interpersonal skills required. Spanish speaking highly preferred! 
 
Compensation: $95,000 – $105,000 Annually
