Locations: Brooklyn, Queens, Bronx, Manhattan, Staten Island
Schedule Options:

  • Morning Shifts: 7:00 AM – 3:00 PM | 8:00 AM – 4:00 PM | 9:00 AM – 5:00 PM
  • Evening Shifts: 2:00 PM – 10:00 PM | 3:00 PM – 11:00 PM | 4:00 PM – 12:00 AM
  • Overnight Shifts: 8:00 PM – 8:00 AM | 11:00 PM – 7:00 AM | 12:00 AM – 8:00 AM
    Note: This client operates 24-hour residential programs.

Position Summary
The Licensed Practical Nurse (LPN) provides comprehensive nursing care and supports individuals attending Day Programs. Under the supervision of a Registered Nurse (RN), the LPN ensures the health, safety, and well-being of participants through clinical care, program implementation, and collaboration with interdisciplinary teams.


Key Responsibilities

  • Administer medications and maintain accurate medication records
  • Monitor health conditions and respond promptly to changes in status
  • Communicate effectively with staff regarding participant needs and concerns
  • Participate in interdisciplinary team meetings and contribute to goal development
  • Implement individualized care and program plans using prescribed methodologies
  • Maintain up-to-date documentation, including progress notes, vitals, and behavioral observations
  • Support incident reporting, grievance responses, and other documentation requests
  • Ensure a clean, safe, and supportive environment
  • Assist with emergency procedures, including evacuation drills and use of specialized equipment
  • Attend screenings, semi-annual reviews, and special team meetings as needed
  • Perform additional duties as assigned

Supervisory Responsibilities

  • Assist the RN in supervising staff involved in medical care
  • May oversee staff administering medications

Qualifications

  • Current New York State LPN license
  • Ability to travel to multiple program locations as needed
