Key Responsibilities:

• Deliver direct physical therapy treatment to adult home care patients.
• Collaborate with Rehab management to coordinate and implement individualized plans of care.
• Maintain timely, accurate documentation and comply with agency, state, and federal policies.

Qualifications:

• Education: Bachelor’s degree in Physical Therapy required; Master’s or Doctor of Physical Therapy (DPT) preferred.
• Experience: At least one year of therapy treatment in a healthcare setting. Home care experience is highly preferred.
• Licensure: Valid driver’s license and access to reliable transportation.
• Skills: Bilingual proficiency in Spanish, Cantonese, or Mandarin is a plus!
