Job Description
Now Hiring: PCP Physician for PACE Program
Location: Evansville, IN
Salary range: $200k-$265k and comprehensive benefits
Hours: Monday – Friday 8:00am-5:30pm
Are you a dedicated and compassionate Physician passionate about improving the quality of life for older adults? Join our client's PACE (Program of All-Inclusive Care for the Elderly) team and play a key role in providing comprehensive care for a diverse and vibrant geriatric population.
What You’ll Do:
- Conduct comprehensive and routine evaluations of PACE participants.
- Manage the care of participants across settings, including hospitals, nursing homes, and outpatient clinics.
- Collaborate with an interdisciplinary team (IDT) to create and implement care plans.
- Provide clinical leadership, coordinate 24-hour care, and participate in family conferences.
- Maintain accurate documentation and stay informed on geriatric best practices.
What You Bring:
- Valid MD license in IN and a degree from an accredited School of Medicine.
- A minimum of 2 years of clinical experience (geriatric experience preferred).
- Knowledge of geriatric medicine and the needs of frail or elderly populations.
- Excellent communication, leadership, and teamwork skills.
