Job Description

Now Hiring: Registered Dietitian for PACE Health Clinic and Day Center
Location: Charleston, SC
Salary range: $70k – $85k and Comprehensive Benefits

We are seeking a compassionate and skilled Registered Dietitian to join our client's team in providing high-quality nutrition care and education to older adults in the PACE (Program of All-Inclusive Care for the Elderly) program. In this role, you will assess, plan, and manage the nutritional care of our participants, collaborating with the Interdisciplinary Team to promote optimal health outcomes.

Key Responsibilities:

  • Conduct thorough nutritional assessments and identify participants’ dietary needs, ensuring proper intake and texture based on individual requirements.
  • Develop and implement personalized nutrition care plans in accordance with American Dietetic Association (ADA) standards, collaborating with participants, families, and the care team.
  • Ensure that meals served meet nutritional standards and maintain proper storage and serving temperatures.
  • Provide in-service education to participants, caregivers, staff, and contractors on nutrition-related topics and special dietary regimens prescribed by the physician.
  • Monitor and evaluate participant nutritional intake and recommend dietary changes as necessary.
  • Work closely with primary care physicians to develop and implement nutrition plans, ensuring effective collaboration with other healthcare providers.
  • Actively participate as a member of the Interdisciplinary Team (IDT), attending meetings and contributing to care planning.
  • Supervise meal service delivery, ensuring compliance with dietary standards and addressing any food service issues with vendors.

Requirements:

  • Bachelor's degree in Nutrition from an ADA-accredited institution.
  • Minimum of 1 year of experience as a Dietitian, preferably working with a geriatric population.
  • Registered member of the ADA and registered by the Commission on Dietetic Registration.
  • Certified Diabetes Educator (CDE) status preferred, but not required; must be CDE eligible within 24 months.
  • Experience with frail or elderly populations preferred; training will be provided if not.
  • Reliable transportation, valid driver's license, and minimum required auto insurance.
  • Ability to pass a comprehensive criminal background check, including education and license verification, reference check, and drug screen.

  • Strong team player with the ability to communicate effectively and work collaboratively with an interdisciplinary team.
  • Excellent analytical thinking and problem-solving skills.
  • Proficient in using basic computer applications and electronic mail.
  • Ability to teach and provide clear nutrition guidance to geriatric participants and their families.
  • Strong attention to detail, with the ability to assess and modify care plans as needed.
