Transition of Care RN Care Manager

Salary: $90,000 – $110,000 per year

We are seeking a dedicated Transition of Care RN Care Manager to join our client's team and play a vital role in facilitating patient care transitions, ensuring continuity and quality of care for our members.

Transition of Care RN Care Manager Overview:

As a Transition of Care RN Care Manager, you will act as a patient advocate, collaborating with inpatient facilities, discharge planners, and case management staff to coordinate seamless transitions of care. Your responsibilities will include:

  • Evaluating patients for transitional planning and/or home health services.
  • Facilitating care coordination through telephonic and on-site collaboration.
  • Ensuring compliance with all applicable laws, regulations, and policies.
  • Serve as the primary point of contact for patients transitioning to home health or other care settings.
  • Collaborate with multidisciplinary teams to develop and execute care plans.
  • Advocate for patients to ensure their needs are met in a timely and effective manner.

Qualifications:

  • Associate’s degree in Nursing required; BSN preferred.
  • At least 1 year of acute, Medical-Surgical nursing experience required.
  • Experience in CHHA/LTHHCP, discharge planning, or utilization management preferred.
  • Current NYS Registered Nurse license and CPR certification.
  • Proficiency in Word and Excel.
  • Bilingual Chinese language skills required.
