Job Description

Position: Bilingual Chinese Community Health Nurse
Location: Manhattan, NY
Organization: Large, Established, and Respected Licensed Home Care Agency (LHCSA)
Reports to: Clinical Supervisor
Salary: $110,000 annually with generous benefits


Role Overview
The Community Health Nurse plays a vital role in the Home Care Team, ensuring quality patient care in accordance with doctors' orders and the plan of care. Serving the Chinese-speaking community, this position involves conducting various in-home assessments and visits on behalf of an MLTC plan and the LHCSA, while working closely with patients, caregivers, and the care management team.


Key Responsibilities

  • Conduct Start of Care, reassessment, post-hospitalization, and UAS visits within required timeframes.
  • Perform clinical and environmental assessments, documenting findings and changes in patients' conditions.
  • Supervise home health aides (HHAs) assigned to cases, providing orientation, instruction, and oversight.
  • Develop, implement, and regularly review the Plan of Care (POC) with patients, caregivers, and HHAs.
  • Assess medication regimens, ensure adherence, and verify related diagnoses for each medication.
  • Coordinate care with other disciplines, such as physical therapy, occupational therapy, MSW, and nutrition as needed.
  • Conduct UAS assessments and Home Assessment Abstracts as requested by MLTC plans.
  • Serve as a liaison between patients and the Care Management Team.
  • Maintain accurate and compliant patient records per local, state, and federal regulations.
  • Precept new nurses and participate in their orientation process.
  • Attend mandatory meetings, in-services, and training sessions.

Qualifications

  • Graduate of an approved school of professional nursing.
  • Current RN license in New York State.
  • Home care experience preferred.
  • Bilingual Chinese language skills required.
  • UAS certification or willingness to obtain certification.
  • Strong verbal and written communication skills.
  • Ability to work both independently and as part of a team.
  • Flexibility and strong prioritization skills.
  • Basic computer proficiency.
