Social Worker – MLTC – Hybrid
As the Social Worker, you will provide direct services to those MLTC clients/families that require assistance with social, economic, long term planning, counseling and/or motivation for increased independence.

Key Responsibilities:

• Performing initial psychosocial assessments and
• Follow-up evaluations of members to determine needs, coping skills, barriers to care and social determinants of health
• Responsible for gathering pertinent social information from member’s families or agency staff and maneuver conferences, etc. 

Qualifications:

• NYS License LMSW
• Master’s degree in Social Work from a school accredited by the Council on Social Work Education and/or the NYS Department of Education
• One year of experience in the provision of social work in an acute care or health care facility/organization
• Experience in a home care organization preferred
• Ability to read, analyze, and interpret general business correspondence, and procedure manuals
• Ability to effectively present information and respond to questions from managers, clients, customers, and the general public. 
• Bilingual Russian required

Compensation:

• Annual Salary: $80,000-$85,000
