Job Title: Social Worker – PACE Program

Location: Oceanport, NJ

Salary: $68,000 – $75,000 per year

Hours: Monday – Friday
8:00am – 5:00pm

About the Social Worker Role:
We are seeking a compassionate and dedicated Social Worker to join the interdisciplinary team at a leading PACE (Program of All-Inclusive Care for the Elderly) program in Oceanport, NJ. In this Social Worker role, you will provide essential social work services to participants and their families, ensuring they receive the support and resources needed to remain in the community while maintaining their health and quality of life.

Key Social Worker Responsibilities:

• Perform initial and periodic assessments (every six months or as needed) of participants, contributing to individualized care plans.
• Deliver case management services, including counseling, education, and advocacy for participants and their caregivers.
• Maintain accurate and timely documentation in compliance with industry standards.
• Develop and lead group counseling sessions and activities to support participant well-being.
• Coordinate with community resources and agencies to meet participants’ needs effectively.
• Provide psychosocial support, address mental health needs, and manage crisis situations as they arise.
• Act as a liaison between participants, families, caregivers, and the interdisciplinary team to ensure clear communication and collaboration.
• Participate in home visits to assess participant environments and provide necessary support.
• Engage in program development, quality improvement initiatives, and policy refinement.

Social Worker Requirements:

• Master’s degree in Social Work (MSW) from an accredited institution.
• Current social work license in the state of New Jersey.
• Two (2) years of experience on an interdisciplinary team in a hospital, nursing home, or community-based setting (preferred).
• Experience working with frail or elderly populations is preferred but not required (training will be provided).
• Reliable transportation, valid driver’s license, and minimum state-required auto insurance.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Successful completion of a comprehensive background check.

Skills and Abilities:

• Strong interpersonal, communication, and conflict resolution skills.
• Proficiency in psychosocial assessment, individual, family, and group counseling.
• Familiarity with the unique needs of frail and chronically ill individuals and their caregivers.
• Competency in maintaining accurate records and preparing clear reports.
• Basic computer skills and comfort with electronic medical records.
