Job Description

Nurse Manager – Inpatient Hospital Case Management
New York, NY
Full-Time | Onsite (Monday – Friday, Business Hours)
Up to $160,000 per year + Generous Benefits

Manager of Inpatient Case Management Overview:
A hospital networks is seeking an experienced Nurse Manager – Inpatient Hospital Case Management to oversee and support the daily operations of the case management team. The manager will be responsible for supervising case managers, ensuring high-quality patient care coordination, facilitating efficient discharge planning, and optimizing resource utilization.
This position requires strong leadership, collaboration with interdisciplinary teams, and active participation in performance improvement initiatives. The Nurse Manager will report to the Director of Case Management and work closely with other hospital departments to ensure compliance with regulatory requirements and effective reimbursement processes.
This is an onsite, full-time position with a Monday through Friday, business hours schedule.

 

Manager of Inpatient Case Management Key Responsibilities:

  • Oversee daily activities of the case management team, including staffing, performance evaluations, and professional development.
  • Support case managers in handling complex patient cases and long-stay cases; escalate to physician advisors as needed.
  • Collaborate with social work supervisors to resolve discharge planning challenges.
  • Review and follow up on avoidable day data to ensure accuracy and efficiency in patient care management.
  • Ensure proper communication and coordination between case managers, physicians, nursing staff, payers, patients, and families.
  • Develop, implement, and maintain professional standards for case management practices.
  • Align case management processes with overall hospital and clinical service objectives.
  • Provide operational oversight to ensure that all activities support efficient, high-quality, and clinically appropriate patient care.
  • Actively participate in hospital-wide performance improvement and quality assurance initiatives.
  • Collaborate with nursing leadership, physicians, and social workers to facilitate interdisciplinary rounds, ensuring appropriate patient management and length of stay.
  • Identify and address barriers to timely discharge and appropriate length of stay through regular unit rounds.
  • Support compliance with hospital policies, regulatory standards, and managed care contract requirements.
  • Participate in hospital and nursing leadership meetings to align case management goals with broader institutional objectives.
  • Provide ongoing education and mentorship to case managers, social workers, and hospital staff.
  • Foster a culture of respect, professionalism, and collaboration among staff, patients, and families.

Manager of Inpatient Case Management Qualifications:

  • Bachelor of Science in Nursing (BSN) required; Master’s degree in nursing or a healthcare-related field preferred (or enrollment in a program).
  • Minimum of 5 years of case management experience and 5-7 years of clinical nursing experience in an inpatient hospital setting.
  • Active RN license in New York State.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA).
