UAS Assessment Nurse (RN)
Compensation
Salary Range: $90,000 – $108,000 per year + $15,000 Sign-On Bonus
Location: Various Territories, NYC
Field-Based / Onsite
Position Summary
As a UAS Assessment Nurse (RN), you will play a vital role in evaluating the clinical, psychological, and social needs of members in the community. This field-based position involves conducting in-home and nursing home assessments using the UAS-NY system, ensuring compliance with New York State Department of Health regulations. You will contribute to developing person-centered care plans and supporting improved patient outcomes.
Responsibilities

- Shift details: Monday – Friday (Business Hours)
- Hours per week: 40
? Key Duties:
- Conduct initial and reassessment visits, determining eligibility and care needs using UAS-NY
- Perform functional, psychosocial, and environmental safety assessments
- Develop and communicate individualized, person-centered service plans
- Educate members and caregivers on program benefits, services, and guidelines
- Submit accurate, timely documentation and communicate urgent needs to care teams
Requirements
? Education
- Required: Associate’s Degree in Nursing
- Preferred (if applicable): Bachelor’s Degree
? Licenses & Certifications
- Required: Active Registered Nurse (RN) license; UAS-NY Certification
? Skills:
- Strong knowledge of community health and managed care practices
- Excellent assessment and clinical evaluation skills
- Proficiency in Microsoft Word and Excel
- Strong communication and organizational skills
