UAS Assessment Nurse (RN)

Full-Time, Permanent Roles
Business Hours, Monday through Friday
$90K – $108K + $15,000 Sign-On Bonus!

Available Territories:

  • Brooklyn – Bilingual Chinese required
  • Westchester – Any language preferred
  • Queens – Bilingual Chinese required
  • Bronx – Bilingual Chinese or Bilingual Spanish required
  • Manhattan – Bilingual Chinese or Spanish required
  • Upstate Counties – Rockland, Putnam, Sullivan, Ulster, Orange, Dutchess

Position Overview

The UAS Assessment Nurse (RN) plays a crucial role in evaluating the clinical, psychological, and social needs of new and existing members. Utilizing the Uniform Assessment System for New York (UAS-NY), you will conduct in-home and nursing home assessments, ensuring compliance with New York State Department of Health (DOH) regulations.

Key Responsibilities

  • Conduct initial and reassessment visits, determining eligibility and care needs
  • Explain program benefits, rules, and services to members and caregivers
  • Perform functional, psychosocial, and environmental safety assessments
  • Develop and communicate individualized person-centered service plans
  • Submit accurate and timely documentation with an error rate below 5%
  • Work within DOH-mandated timeframes (minimum two assessments per day)
  • Identify and communicate urgent care needs to the care management team and PCP

Required Qualifications

  • Associate’s Degree in Nursing (Bachelor’s preferred)
  • Minimum one year of experience in Community Health, Managed Care, or a related field
  • UAS-NY certified
  • Proficiency in Microsoft Word & Excel
