Job Description
Nurse Practitioner – Geriatric Primary Care
Field based
Queens, NY (occasional Brooklyn coverage)
$140k – $150k + Sign on Bonus!
Car and driver license required
Are you a compassionate and skilled Nurse Practitioner looking to make a meaningful impact in the lives of at-risk elderly patients? We are seeking an experienced Nurse Practitioner to join our Interdisciplinary Co-Management Program, providing comprehensive primary care services to a designated panel of members in the community.
Key Responsibilities:
- Conduct comprehensive assessments addressing both physical and psychosocial needs
- Develop personalized care plans aligned with patient health goals
- Provide self-management education and tools to improve patient outcomes
- Coordinate preventive care services and ensure access to necessary resources
- Serve as a clinical resource for patients, families, and interdisciplinary teams
What We’re Looking For:
- Master’s Degree in Nursing; graduate of an accredited Nurse Practitioner program
- Active NYS RN/NP License, Registration, & DEA certificate
- Minimum 1 year of Nurse Practitioner experience, preferably in geriatric care
- Clinical background in acute, subacute, or long-term care is a plus
