Job Description

Nurse Practitioner – Palliative Care (Manhattan & Brooklyn)
$125K – $145K + $15K Sign-On Bonus
Field-Based | Nursing Homes & Residences

Are you a compassionate and skilled Nurse Practitioner with expertise in palliative care, symptom management, and hospice eligibility? What You’ll Do:

  • Conduct comprehensive patient assessments and physical exams
  • Evaluate hospice eligibility and facilitate patient transitions
  • Lead goal-setting discussions with patients, families & care teams
  • Implement palliative care plans to manage pain and symptoms
  • Collaborate with physicians and interdisciplinary teams
  • Provide family education & support on disease progression and care options

What We’re Looking For:

  • Master’s degree in Nursing & completion of an approved Nurse Practitioner program
  • Active NYS RN & Nurse Practitioner licensure (or eligibility)
  • 1+ years of clinical Nurse Practitioner experience (Palliative Care, Hospice, or Geriatrics preferred)
  • CPR-BLS & valid NYS Driver’s License preferred
