Our client, a leading provider in the PACE (Program of All-Inclusive Care for the Elderly) space, is looking for a mission-driven RN Care Manager to join their compassionate, interdisciplinary team. This role offers the opportunity to deliver holistic care—combining clinical expertise with coordination, advocacy, and heart.
As a key member of the care team, you’ll assess participant needs, implement care plans, and serve as the nursing voice on a collaborative IDT. You’ll work across clinic, home, and facility settings, ensuring that each participant receives personalized, high-quality care that promotes dignity, autonomy, and optimal wellness.
What You'll Do

• Deliver direct nursing care across a variety of settings: clinic, participant homes, and contracted facilities.
• Participate in 24/7 triage rotation, ensuring timely response and support for participants.
• Collaborate with physicians, social workers, therapists, and home care staff to develop and implement comprehensive care plans.
• Monitor and manage acute and chronic care needs.
• Coordinate specialist referrals, hospital discharges, and medication management.
• Educate participants and caregivers on medication, lifestyle, and safety practices.
• Lead and participate in interdisciplinary team meetings, family meetings, and utilization reviews.
• Delegate and supervise aides and support staff as needed.

What We’re Looking For

• RN license (required); BSN preferred.
• Minimum 1 year of RN experience; home care, long-term care, or managed care background highly preferred.
• Prior experience working with frail or elderly populations a plus—but not required. Training will be provided.
• A collaborative spirit and a deep understanding of person-centered care.
• Strong critical thinking, adaptability, and commitment to quality.
• Familiarity with EMR systems and Microsoft Office Suite.
• Must have reliable transportation, valid driver’s license, and current immunizations.
