Peer Specialist / Field-Based / NYC

We’re currently recruiting for a full-time Peer Specialist to join a respected NYC health plan that’s been supporting the community for over 30 years. • $51,500 salary + excellent benefits (including pension plan)
• Full-time | 100% field-based
About the Role:
The Peer Specialist will provide hands-on support to members living with Serious Mental Illness (SMI) and/or Substance Use Disorders (SUD), helping them regain independence and reach recovery goals through lived experience, mentorship, and advocacy.
Key Responsibilities:
• Serve as a role model and support members in both outpatient and inpatient settings
• Conduct 1:1 sessions to help clients define personal recovery goals and action plans
• Assist with supportive housing applications and escort members to housing interviews
• Support transportation coordination and accompany members to appointments
• Collaborate with the Behavioral Health Care Team and attend clinical meetings
• Use lived experience to help clients overcome stigma, build self-advocacy skills, and develop coping strategies
• Provide peer support during times of crisis and emergency department overuse
• Empower clients through community resource access and life skills coaching
Qualifications:
• High School diploma or GED required
• At least 5 years of experience in behavioral health or a related combination of education and experience
• NYS Peer Specialist Certification required (Provisional status accepted as a pipeline option)
• Knowledge of recovery processes and challenges associated with SMI/SUD
• Strong communication and problem-solving skills
• Tech-savvy with Microsoft Office and EMR documentation skills
• Bilingual (English/Spanish) a plus
Preferred but not required:
• CASAC-T certification
• Experience supporting individuals under probation or parole
