MLTC Intake Nurse – RN – (Bronx, NY) – Bilingual Spanish or Chinese
Compensation: $95,000 – $105,000 Annually  

As a MLTC-Intake RN, you will be responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their family. 
 
Responsibilities:
The Intake RN will be responsible for knowing outreach activities within the community and the assessment of eligibility and safety of potential members. Collaborate with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members. Communicate with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.
 
Qualifications:
Must be a self-assured professional with a current New York State Registered Nurse License required. Bachelor’s degree in Nursing required. One (1) year experience in an acute care setting required. Must have UAS- NY experience.  Home care and management experience preferred. MS Office required. Effective oral, written, communication and interpersonal skills required. Bilingual Spanish or Chinese speaking preferred.
