Key Responsibilities

  • Conduct comprehensive clinical assessments in the home, including physical, psychosocial, and functional evaluations.

  • Develop and update individualized care plans using evidence-based guidelines.

  • Coordinate with Care Managers and IDT members to ensure timely follow-up, appropriate services, and continuity of care.

  • Facilitate safe transitions from hospitals, subacute rehab, or nursing facilities using the modified Coleman Model.

  • Order diagnostic tests, therapeutic services, and preventive care interventions as appropriate.

  • Reconcile medications, monitor polypharmacy, and optimize pharmaceutical management.

  • Evaluate the need for acute inpatient care and communicate findings with collaborating physicians.

  • Educate members and families on disease management, preventive care, and end-of-life planning when appropriate.

  • Participate in interdisciplinary team meetings and serve as a clinical resource to staff.

  • Maintain accurate, timely documentation in EMR systems.

Qualifications

  • Master’s Degree in Nursing and graduation from an accredited Nurse Practitioner program.

  • Active NYS RN / NP license, registration, and DEA certificate required.

  • Minimum 1 year of NP experience, including geriatric care.

  • Prior experience as an RN in acute, subacute, or long-term care strongly preferred.

  • Managed care experience is a plus.

  • Strong communication, independent decision-making, and care coordination skills.

  • Comfort working field-based with regular home visits (reliable vehicle required).

Why This Opportunity

  • Competitive salary $140k–$150k plus $15,000 sign-on bonus.

  • Mission-driven organization dedicated to high-risk older adults.

  • Independent, field-based role with strong interdisciplinary support.
