Job Description

Position: Registered Nurse / RN / Assisted Living Facility / Float NYC
Location: Covering Brooklyn, Bronx, Queens, Manhattan, and Staten Island
Schedule:
7:00 a.m. – 3:30 p.m.  Monday through Friday or 
11:30 a.m. – 8:00 p.m. Monday through Friday or 
Extended shifts (up to 4 days per week): 7:00 a.m. – 5:00 p.m.

Overview:

The RN Float will provide skilled nursing care across assisted living facilities in all five boroughs, ensuring high-quality, patient-centered care.


Key Responsibilities

  • Conduct initial and ongoing patient assessments, including the use of Oasis tools when needed.
  • Collaborate with families, physicians, and supervisors to develop and implement care plans.
  • Deliver skilled nursing care per the Plan of Treatment, including preventive, rehabilitative, and specialized procedures.
  • Observe and report changes in patients' conditions to the supervisor or care coordinator promptly.
  • Document all care provided and maintain compliance with organizational policies.
  • Provide education and support to patients and caregivers.
  • Supervise and orient home health aides as needed.
  • Respond to emergencies within assisted living facilities professionally and promptly.
  • Participate in case management, in-service training, and performance improvement activities.

Qualifications

  • Valid NYS RN License (Required).
  • 1+ year of nursing experience, preferably in geriatrics or home health (Preferred).
  • Strong clinical skills with the ability to manage multiple priorities.
  • Vehicle required for travel across boroughs.
  • Ability to commute for orientation in Brooklyn and cover all five boroughs.

Compensation and Benefits

  • Salary: $120,000 – $125,000 annually and $15,000 sign on bonus
