Our client is a Certified Home Health Agency that provides skilled home services, including nursing, physical therapy, occupational therapy, speech therapy, and social work.

As a Home Care RN, you will be responsible for conducting clinical assessments and developing, overseeing, and implementing patient care plans in collaboration with patients, families, and physicians to ensure safe and effective care at home. You will also monitor and adjust care plans as necessary, provide oversight of Home Health Aide services, and participate in the interdisciplinary care delivery process.

Qualifications:

• Current NYS RN License and registration
• Associate’s Degree in Nursing Required. Bachelor’s Degree in Nursing Preferred
• Minimum of 2 years of experience in assessing patients for home care safety and developing care plans
• Minimum of 1 year of supervisory experience in a hospital or home care agency
• Patient Review Instrument (PRI) assessment experience preferred
• Valid driver’s license and proficiency in MS Office required
• Bilingual skills are a plus, but not required

Compensation:

• Annual salary: $105,000 – $110,000
