Hospice Field Admissions Nurse (RN)
Compensation
Salary Range: $112,000 – $130,000 per year
Location: Brooklyn, Queens, and Nassau County, NY
Onsite / Field-Based
Position Summary
As a Hospice Field Admissions Nurse (RN), you will provide compassionate, patient-centered care by conducting initial assessments and facilitating hospice admissions for patients across home, hospital, and facility settings. This role focuses on developing individualized care plans, ensuring compliance with hospice standards, and supporting patients and families during end-of-life care. You will also engage with community providers to support program awareness and performance improvement within a mission-driven hospice and palliative care program.

• Key Duties:

• Perform physical and psychosocial assessments to evaluate hospice eligibility
• Develop individualized plans of care, goals, and recommendations
• Admit patients to hospice services in accordance with NHPCO standards
• Conduct visits in home, hospital, and skilled nursing facility settings
• Collaborate with interdisciplinary teams to ensure coordinated patient care
• Engage in outreach with physicians and providers to promote hospice services
• Document assessments and care plans accurately and timely

Requirements
• Education

• Required: Graduate of an accredited School of Nursing
• Preferred: Bachelor of Science in Nursing (BSN)

• Licenses & Certifications

• Required: Active New York State Registered Nurse (RN) license; valid NYS Driver’s license

Skills:

• Strong clinical assessment and hospice care planning skills
• Effective communication and patient/family engagement abilities
• Ability to work independently in a field-based environment
• Proficiency in documentation and basic computer applications (Word, Excel)
