RN Care Manager (PACE Program)
Crestwood, IL

$87,000 – $95,000
About the Opportunity
I’m partnering with a mission-driven healthcare organization seeking an experienced RN Care Manager to join their interdisciplinary team supporting a vulnerable, elderly population. This is a highly impactful role combining clinical care, care coordination, and community-based nursing within a PACE (Program of All-Inclusive Care for the Elderly) model.

Key Responsibilities

Provide high-quality nursing care across settings including clinic, center, and patient homes

Assess, develop, and implement individualized care plans in collaboration with the interdisciplinary team (IDT)

Coordinate and monitor acute and chronic care needs for assigned participants

Support transitions of care across home, hospital, assisted living, and skilled nursing settings

Conduct nursing assessments and ensure timely, accurate documentation
-
Educate patients, families, and caregivers on medications, self-care, and disease management

Collaborate with providers and specialists to ensure continuity of care

Delegate and oversee tasks for CNAs and home care aides

Participate in on-call rotation for triage support
-
Engage in quality improvement initiatives and utilization review activities
Qualifications
-
Active RN license (Illinois) required

Bachelor’s degree in Nursing (BSN) preferred
-
Minimum 1+ year of RN experience required

Experience with elderly populations, home care, long-term care, or managed care strongly preferred
-
BLS certification (or ability to obtain within 90 days)
