Job Title: Social Worker, LICSW – CATCH Program
Compensation
Salary Range: $86,000 – $104,833 per year
Location: Boston, MA – Onsite
Position Summary
As a Social Worker, LICSW within the CATCH (Care Coordination for Children with Complex Health Needs) Program, you will provide clinical support, case management, and behavioral health services to medically complex pediatric patients and their families. Working as an integral member of an interdisciplinary care team, you will conduct psychosocial assessments, coordinate care plans, provide counseling services, and advocate for patients and families navigating complex healthcare, educational, and community systems. This role offers the opportunity to make a meaningful impact on the lives of children with chronic illnesses and disabilities.

Key Duties:
- Conduct comprehensive psychosocial assessments, develop treatment plans, and provide case management services for medically complex pediatric patients and their families
- Provide individual, family, and group counseling, psychoeducation, behavioral health support, and crisis intervention services
- Coordinate care with healthcare providers, schools, community agencies, behavioral health professionals, and family members to ensure continuity of care
- Support families through guardianship processes, transitions to adult care, special education services, and access to community resources and benefits
- Participate in interdisciplinary team meetings, maintain accurate documentation within the electronic medical record, and advocate for patient and family needs
Requirements
Education
- Required: Master’s Degree in Social Work and LICSW Licensure
- Preferred (if applicable): Community health or pediatric-focused experience
Licenses & Certifications
- Required: Licensed Independent Clinical Social Worker (LICSW) in Massachusetts
- Additional certifications (if applicable): N/A
Skills:
- Experience or strong interest in working with pediatric patients, families, chronic illness, and disability populations
- Strong clinical assessment, counseling, crisis intervention, and case management skills
- Excellent communication, collaboration, organizational, and advocacy abilities
- Bilingual English/Spanish preferred
