Job Description

Job Title: RN Case Manager – Behavioral Health

Compensation 
Salary Range: $100,000 – $135,000 per year DOE

Location: Long Island, NY – Hybrid / Field-Based

Position Summary

As an RN Case Manager – Behavioral Health, you will provide comprehensive care coordination, assessment, planning, advocacy, and service authorization to support members across the continuum of care. This role is responsible for ensuring members receive appropriate services, promoting quality and cost-effective outcomes, and collaborating with families, providers, and interdisciplinary teams. Through both telephonic and in-person interactions, you will support members in maintaining the highest level of independence while addressing their medical, behavioral health, and psychosocial needs.

Key Duties:

  • Conduct assessments, care planning, and ongoing care management to ensure continuity of care across hospital, sub-acute, long-term care, and home settings
  • Coordinate services and collaborate with members, caregivers, physicians, and interdisciplinary teams to address healthcare needs and goals
  • Perform home visits to evaluate living conditions, functional status, cognitive needs, and community support requirements
  • Develop and maintain comprehensive Patient-Centered Service Plans while monitoring member eligibility and service utilization
  • Support discharge planning, care transitions, grievance resolution, and documentation activities in accordance with organizational and regulatory requirements

Requirements

Education

  • Required: Associate Degree in Nursing from an accredited nursing program
  • Preferred (if applicable): Bachelor’s Degree in Nursing

Licenses & Certifications

  • Required:
    • Current, unrestricted New York State Registered Nurse (RN) license
    • Valid driver’s license, vehicle, and verifiable insurance
  • Additional certifications (if applicable): Case Management Certification preferred

Skills:

  • Strong knowledge of case management principles, healthcare regulations, and care coordination practices
  • Excellent assessment, critical thinking, decision-making, and problem-solving skills
  • Ability to communicate effectively with culturally diverse populations and multidisciplinary teams
  • Proficiency with Microsoft Office, including Outlook, Word, and Excel
  • Strong organizational, time management, and member advocacy skills
  • Bilingual speaking and writing skills preferred
