Job Description
Locations: Brooklyn, Bronx, Queens, Manhattan, Staten Island, Long Island
Setting: Residential Programs (including but not limited to group homes and supported living environments)
Schedule: Full-time | Part-time | Per Diem
Position Summary
We are seeking compassionate and experienced Registered Nurses (RNs) to join our interdisciplinary team supporting individuals in residential programs. The RN will coordinate healthcare services, provide clinical oversight, and ensure the delivery of high-quality, person-centered care.
Key Responsibilities
- Serve as a member of the interdisciplinary team, coordinating and following up on healthcare services for assigned programs
- Arrange medical services and issue referrals for individuals as needed
- Provide follow-up care for medical concerns and monitor health outcomes
- Conduct staff training on medical administration, hygiene, infection control, first aid, and other health-related topics
- Manage infection control protocols, including identification, assessment, reporting, and staff education
- Participate in team meetings, annual reviews, and individual case conferences
- Monitor staff health records and ensure compliance with regulatory testing requirements
- Provide immediate first aid and coordinate emergency care
- Maintain accurate and up-to-date medical documentation for each individual
- Administer medications and supervise certified staff in medication administration
- Oversee medication storage and ensure compliance with safety standards
- Supervise and mentor Licensed Practical Nurses (LPNs) and less experienced RNs
- Maintain the nursing office and manage medical supply inventory
- Transport individuals to medical appointments as needed
- Conduct home visits and follow up on care recommendations
- Review visit summaries and ensure implementation of physician recommendations
- Ensure Medication Administration Records (MARs) are accurate and that AMAPs administer medications as prescribed
- Conduct member assessments and provide in-services as needed
- Address additional health-related needs to support individuals’ care plans
- Stay current with nursing best practices and regulatory changes
- Perform other related duties as assigned
Qualifications
- Current and valid Registered Nurse (RN) license in the State of New York
- CPR/BLS certification
- Graduate of an accredited nursing program
- At least 1 year of experience providing community-based health services to individuals with developmental disabilities preferred
- Strong communication, organizational, and interpersonal skills
- Ability to maintain confidentiality and work collaboratively with staff, individuals, families, and external contacts
- Familiarity with adaptive equipment and assistive devices
- Willingness to work across various program sites
- Valid New York State Driver’s License preferred but not required
