Job Description

An Assisted Living Program in the Bronx, is currently seeking a full time Nurse Manager to join their team!

Job Summary: The Nurse Manager of the Assisted Living Program (ALP) is responsible for ensuring clinical excellence in the coordination and provision of care to the residents of the ALP, and in ensuring compliance with established policies and procedures, consistent with DOH, OMIG and OSHA regulations. The Nurse Manger reports to the Director of the Assisted Living Program.

DUTIES & RESPONSIBILITIES:

  • Responsible for planning, coordinating and delivering care in the daily operations of the wellness center.
  • Responsible for scheduling nursing staff to ensure the continuity of nursing care 24 hours a day, 7 days a week.
  • Ensures that resident assessments (UAS) are done in an accurate and timely fashion, according to DOH regulations.
  • Ensures that the Individualized Service Plans (ISP) for residents are done in an accurate and timely fashion, according to DOH regulations.
  • Directs and supervises nursing staff who work in the ALP (RNs/LPNs, HHAs) to ensure the resident plan of care is implemented appropriately.
  • Ensures that residents are seen by a physician (either through the Wellness Center or by their private physician in the community) as needed, but at least every 6 months or upon significant change, and that the Medical Evaluation form is completed in an accurate and timely fashion, according to DOH regulations.
  • Ensures that an initial mental health evaluation (MHE) is completed prior to admission for all residents with mental health diagnoses or a psychotropic medication order.
  • Ensures that physician orders are obtained and carried out as needed according to the ISP.

SPECIALIZED SKILLS AND COMPETENCIES:

  • Demonstrated ability to perform skilled nursing tasks including all relevant nursing care procedures
  • Acts as a professional role model for community staff
  • Excellent verbal and written communication skills
  • Demonstrates ability to work with computers, including electronic medical record.
  • Ability to assess, plan, and intervene in resident care activities appropriate to the adult and elderly population.

QUALIFICATIONS:

  • Currently licensed to practice as a Registered Professional Nurse in New York State,
  • Bachelors Degree in Nursing, preferred
  • 4 years nursing experience preferred, with at least one year of managerial experience.
  • BCLS certification
  • UAS Certified, or able to become certified
