Job Description

Primary Care Nurse Practitioner (PACE Program)

Location: Oceanport, NJ
Schedule: Monday–Friday, business hours
Reports to: Medical Director 
Position Type: Full-time
Salary: $130,000

About the Role:

We are seeking a dedicated and experienced Nurse Practitioner to join our client’s Program of All-Inclusive Care for the Elderly (PACE) team. This Nurse Practitioner role offers the unique opportunity to provide comprehensive care to medically complex and frail older adults across clinical and home-based settings.

As the Nurse Practitioner, you’ll manage direct and indirect care for participants in both the PACE health clinic and day center, working closely under the supervision of the Medical Director. This is a collaborative and highly autonomous position that plays a key role in participant care and interdisciplinary care planning.

Key Responsibilities:

  • Conduct health histories, physical exams, and participant teaching

  • Order, interpret, and follow up on lab tests and diagnostic results

  • Initiate, evaluate, and adjust medications and treatments within approved protocols

  • Coordinate daily care of participants in collaboration with the PCP

  • Provide referrals to specialists and request consults as needed

  • Serve as a liaison between healthcare providers, team members, and families

  • Participate in the PACE provider on-call schedule

  • Evaluate and treat participants in clinics, homes, ALFs, SNFs, and hospitals as medically indicated

  • Support advance care planning and complete DNR orders when appropriate

  • Document participant changes in the EMR and communicate updates with the care team

  • Precept medical students and residents during PACE rotations

  • Attend staff meetings and contribute to interdisciplinary care planning

Qualifications:

  • Master’s degree from an accredited Nurse Practitioner program

  • Active RN license and Family Nurse Practitioner (FNP) certification in the state of New Jersey

  • 3–5 years of nursing experience preferred; NP experience strongly preferred

  • At least 1 year of experience working with frail or elderly populations preferred (training provided if not met)

  • Valid driver’s license, reliable transportation, and current auto insurance

  • Up-to-date immunizations and medical clearance for communicable diseases

  • Successful completion of background check and all credential verifications
