Job Description
CHHA Field Registered Nurse (Union) – Manhattan
Full-Time | $49.36 /hr + Benefits
A well-established nonprofit healthcare organization serving older adults in NYC is seeking a CHHA Field RN to provide skilled nursing care in patients’ homes throughout Manhattan.
Responsibilities:
- Conduct OASIS assessments and routine home visits
- Develop and implement individualized plans of care
- Supervise HHAs and coordinate interdisciplinary services
- Monitor patient status and manage transitions of care
- Ensure Medicare and regulatory compliance
Requirements:
- Active NYS RN license
- 1+ year hospital or CHHA experience preferred
- OASIS and Medicare PPS knowledge strongly preferred
