• Field-based position serving patients throughout Westchester County

Key Duties

• Perform in-home nursing assessments to determine patient needs, eligibility, and appropriate plans of care.
• Develop, implement, monitor, and update individualized care plans based on patient needs and progress.
• Provide skilled nursing care, including wound care, medication management, disease education, and other clinical services.
• Coordinate care and communicate with patients, families, physicians, home health aides, and interdisciplinary team members.
• Complete accurate and timely clinical documentation using HCHB or other electronic medical record systems.
• Identify the need for additional services, including PT, OT, speech therapy, and social work, and coordinate referrals as appropriate.
• Supervise and provide guidance to home health aide personnel as needed.
• Participate in case conferences and ensure compliance with agency policies, infection control standards, and applicable regulatory requirements.

Requirements
Education

• Graduate of an accredited nursing program.

Licenses & Certifications

• Current and unrestricted New York State Registered Nurse (RN) license.
• Current physical examination as required for employment.

Experience

• Minimum of 6 months of clinical/hospital nursing experience.
• Home care or CHHA experience is preferred.
• HCHB experience is a plus.
• Vehicle required for most territories.

Skills

• Strong clinical assessment and care coordination skills.
• Excellent communication, organization, and documentation abilities.
• Ability to work independently in a field-based home care environment.
• Comfortable using electronic medical record systems; familiarity with HCHB is preferred.
• Ability to collaborate effectively with patients, families, physicians, and interdisciplinary care teams.
