Full-Time Temporary Field Registered Nurse

Hourly Rate: $48 – $50
Location: 1200 Waters Place Bronx New York 10461 United States

Position Summary: 
The ADHC/ CABAPLE RN will be an integral part of a new community based project that focuses on reducing hospitalization, re-hospitalization and long -term care placement through assessment of the participant’s home environment. The successful candidate will work closely with an OTR to screen adults in the community for enrollment in the CAPABLE program. The RN will assess the implications and complications related to the participant’s acute and chronic medical conditions, activities of daily living, functional status, medication regimen and how these factors impact on the participant’s ability for safety and independence in the home. He/she will assist in evaluating the home setting and recommend interventions that will maximize the participant’s independence and safety for daily activities. The RN will work with the participant in developing a unique approach to maximize self-reporting techniques pertinent to the participant’s health and well-being and overall quality of life. The Registered Nurse will complete a 10 hr virtual training program focused on assessment, planning, implementation and evaluation of CAPABLE services.

Responsibilities:
Key Duties

  • Conducts comprehensive assessments in the home environment identifying factors that limit over-all functional status, independence and quality of life
  • Evaluates participant’s physical, cognitive, emotional and sensory abilities
  • Evaluates participant’s abilities to perform activities of daily living(ADL’s) and provides recommended interventions ·
  • Assesses home environment and recommends modifications to improve safety and accessibility ·
  • Completes required screens, evaluations, progress notes and attendance documentation according to CAPABLE requirements ·
  • Coordinates with CAPABLE Team on plans and goals that contribute to Quality Improvement initiatives ·
  • Provide education to participants and their caregivers regarding disease management, medication adherence and safety protocols as they relate to CAPABLE requirements.
  • Maintains and coordinates schedule for RN assessments, ensures timely arrival to appointments ·
  • Maintains RN statistical reports ·
  • Maintain accurate, timely, and compliant medical records for every participant encounter in accordance with New York State Department of Health (NYSDOH) CAPABLE regulations and agency policy ·
  • Coordinates care with OTR, and other community resources to ensure holistic participant support ·
  • Assists in ADHC Nursing support according to schedule ·
  • Maintains professional competence by attending appropriate, special courses and seminars ·
  • Adhere to all federal, state, and local regulations, including maintaining strict adherence to infection control protocols and HIPAA privacy standards
  • Communicates regularly with Project Coordinator

Requirements:
Education

  • Graduate of an accredited nursing program.

Licenses/Certifications

  • Must hold a current, active, and unrestricted license to practice as a Registered Professional Nurse (RN) in New York State.

Experiences Needed

  • Minimum of [2] years of nursing experience in home health care/CHHA, and community health setting,.

Skills

  • Strong clinical assessment, critical thinking, and communication skills. Ability to work independently with minimal supervision. ·
  • Proficiency in electronic medical record (EMR) documentation systems. · willingness to travel to participant’s home within the assigned service area.

Age Related Skills and Competencies:

  • 5 years experience working with senior population ·
  • Ability to work independently while effectively collaborating with team
  • Excellent communication, organization, clinical reasoning and time-management skills
  • Cultural sensitivity and respect for participant’s preferences ·
  • Willingness and cooperation to perform other functions related to ADHC Program

Specialized Skills and Competencies:

  • Ability to manage a flexible schedule ·
  • Bi-lingual preferred Physical Requirements ·
  • Ability to stand, walk, bend, and lift patients or equipment as required to provide safe and effective care. ·
  • Ability to navigate various home environments that may not be ADA-accessible.
