CHHA Field Registered Nurse (Union)
Compensation
Hourly Rate: $49.36 per hour
Location: Manhattan, NY

Position Summary
As a CHHA Field Registered Nurse (Union), you will provide skilled nursing care to patients in their homes throughout Manhattan within a well-established nonprofit healthcare organization serving older adults. This role focuses on delivering high-quality, patient-centered care while ensuring compliance with Medicare regulations and organizational standards. You will collaborate with interdisciplinary teams to support care planning, ensure continuity of care, and optimize patient outcomes in a home care setting.
Responsibilities

• Key Duties:
- Conduct OASIS assessments and perform routine home visits
- Develop and implement individualized plans of care for patients
- Supervise Home Health Aides (HHAs) and coordinate interdisciplinary services
- Monitor patient status and manage transitions of care
- Ensure compliance with Medicare regulations and applicable standards
Requirements
• Education
- Required: Associate or Bachelor’s degree in Nursing
• Licenses & Certifications
- Required: Active New York State Registered Nurse (RN) license
Skills:
- Knowledge of OASIS assessments and Medicare PPS guidelines preferred
- Strong clinical assessment and care planning skills
- Ability to work independently in a field-based home care setting
- Effective communication and coordination with interdisciplinary teams
