Job Title: Social Worker – Oncology

Compensation
Hourly Rate: $47.57 – $58.80 per hour, depending upon experience

Location: Nassau County, Long Island, NY – Onsite

Position Summary

As an Oncology Social Worker, you will serve as a key member of a multidisciplinary oncology care team, providing comprehensive psychosocial support to patients and families receiving treatment in Medical Oncology and Radiation Oncology. This role addresses the emotional, social, financial, and practical challenges associated with cancer diagnosis and treatment while ensuring patients receive compassionate, coordinated, and patient-centered care throughout their oncology journey.

Key Duties:

• Conduct comprehensive psychosocial assessments and develop individualized care plans addressing emotional, social, and practical needs of oncology patients
• Provide short-term counseling, crisis intervention, supportive counseling, and assistance with adjustment to diagnosis, treatment, recurrence, and end-of-life concerns
• Coordinate referrals to community resources, behavioral health services, support groups, home care, hospice, and specialty services
• Collaborate with oncologists, radiation oncologists, nurse navigators, financial counselors, palliative care teams, and other interdisciplinary providers to optimize patient outcomes
• Support distress screening programs, survivorship planning initiatives, advance care planning discussions, and quality improvement efforts while maintaining accurate documentation in the electronic health record

Requirements

Education

• Required: Master of Social Work (MSW) from an accredited program
• Preferred (if applicable): N/A

Licenses & Certifications

• Required: Current New York State LMSW or LCSW License
• Additional certifications (if applicable): Oncology Social Work Certification (OSW-C) required within 12 months of hire

Skills:

• Strong knowledge of oncology treatment modalities, including medical and radiation oncology
• Experience with crisis intervention, psychosocial assessments, and short-term therapeutic counseling
• Excellent communication, care coordination, and patient advocacy skills
• Ability to manage complex emotional, social, and financial issues within a fast-paced outpatient oncology environment
