Job Description

Job Title: Pediatric Field RN / Coordinator of Care
Compensation
Salary Range: $105,000 – $110,000 annually
Sign-On Bonus: $15,000

Position Summary
A large, highly respected Certified Home Health Agency (CHHA) is seeking a Pediatric Field RN / Coordinator of Care to join its growing team. In this role, you will manage all aspects of patient care for pediatric home care patients, including assessments, care planning, coordination of services, and ongoing clinical oversight. This is a full-time, permanent opportunity offering excellent compensation, comprehensive benefits, and strong work-life balance.
Responsibilities
Schedule / Shift

  • Monday – Friday
  • Full-Time, Permanent Position
  • Field-based throughout Queens, NY

Key Duties

  • Conduct home care assessments to determine eligibility and develop individualized care plans
  • Provide skilled nursing services, including medication management, wound care, and patient/family education
  • Coordinate care with physicians, families, interdisciplinary team members, and community resources
  • Complete clinical documentation accurately and timely within HCHB or other EMR systems
  • Supervise and orient home health aides as needed
  • Identify needs for additional services including PT, OT, Speech Therapy, and Social Work
  • Monitor patient progress, update care plans, and participate in case conferences
  • Ensure compliance with infection control standards, regulatory requirements, and agency policies

Requirements
Education

  • Required: Graduate of an accredited Nursing program

Licenses & Certifications

  • Active New York State Registered Nurse (RN) license required
  • Current physical examination required

Experience & Skills

  • Minimum of 6 months of clinical nursing experience required
  • Experience in acute care, SNF, LTC, LTAC, or home care settings accepted
  • Pediatric nursing experience strongly preferred
  • Experience with EMR systems required
  • HCHB experience preferred
  • Strong assessment, communication, care coordination, and documentation skills
  • Ability to work independently in a field-based environment
