Job Title: Weekend Field RN / Coordinator of Care
Salary Range: $105,000 – $110,000 per year
Sign-On Bonus: $15,000

Position Summary
A large, respected Certified Home Health Agency (CHHA) is seeking a Weekend Field RN / Coordinator of Care to provide comprehensive nursing care and care coordination services to patients in their homes. This role is responsible for conducting patient assessments, developing and managing care plans, coordinating services with interdisciplinary teams, and ensuring quality patient outcomes. The ideal candidate is a compassionate, organized Registered Nurse who enjoys working independently while making a meaningful impact in the community.
Responsibilities
• Schedule / Shift
- Friday, Saturday, and Sunday
- 13-hour shifts
- Full-time permanent position
• Key Duties
- Conduct home care assessments to determine eligibility and develop individualized care plans.
- Provide skilled nursing services, including wound care, medication management, and patient education.
- Coordinate care with patients, families, physicians, and interdisciplinary healthcare teams.
- Complete clinical documentation accurately and timely within agency guidelines.
- Supervise and orient home health aides as needed.
- Identify needs for additional services, including PT, OT, Speech Therapy, and Social Work.
- Monitor patient progress and adjust care plans as appropriate.
- Participate in case conferences and interdisciplinary care planning.
- Ensure compliance with infection control standards, regulatory requirements, and agency policies.
Requirements
• Education
- Required: Graduate of an accredited Registered Nursing program.
• Licenses & Certifications
- Required: Active New York State Registered Nurse (RN) License.
Skills
- Minimum of 6 months of clinical or hospital nursing experience.
- Strong assessment, care planning, and patient education skills.
- Experience with EMR systems; HCHB experience preferred.
- Excellent communication, organizational, and documentation skills.
- Ability to work independently in a field-based environment.
- Reliable vehicle required for most service territories.
- Current physical examination required.
