Job Description
Job Title: LPN – HEDIS and Population Health Management Specialist
Compensation
Salary Range: $75,000 – $85,000 per year
Location: Midtown Manhattan, NY
Hybrid (3 days onsite / 2 days remote)
Position Summary
As an LPN – HEDIS and Population Health Management Specialist, you will support HEDIS and Population Health initiatives by identifying opportunities to improve member health outcomes, close care gaps, and increase utilization of preventive health services. This role involves data collection, analysis, provider and member outreach, health education, and reporting activities to ensure compliance with TRICARE, NCQA HEDIS, and organizational requirements. You will collaborate with internal and external stakeholders to develop and implement population health strategies that improve quality metrics and overall community health.

- Shift details: Hybrid Schedule (3 days onsite / 2 days remote)

Key Duties:
- Conduct provider and member outreach to support HEDIS compliance, care gap closure, and population health initiatives
- Collect, retrieve, analyze, and maintain healthcare data and medical records in accordance with HEDIS specifications
- Develop and maintain reporting tools, analyze trends, and recommend quality improvement initiatives
- Support disease prevention, health promotion, and patient education programs using evidence-based practices
- Collaborate with internal teams, providers, vendors, and community partners to improve health outcomes and program performance
Requirements
Education
- Required: High School Diploma or equivalent
- Preferred (if applicable): Associate Degree in a health-related field
Licenses & Certifications
- Required: N/A
- Additional certifications (if applicable): Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) preferred; Certified Health Education Specialist (CHES) preferred
Skills:
- Strong knowledge of HEDIS measures, population health management, and preventive healthcare practices
- Understanding of medical terminology, anatomy, physiology, ICD-10, CPT, and HCPCS coding
- Strong analytical, data management, and problem-solving skills
- Proficiency with Microsoft Office applications including Excel, Word, Outlook, and PowerPoint
